A new form can be created in multiple ways, from the Exam's Design tab or from within an item bank. See the Building an Exam Form article for how to create a form from an item bank.
Follow the steps below to create a new form from the Exam.
- Log into your Prolydian account at https://app.prolydian.com and go to Exams > Exams
- Click on the exam
- Click on the Design tab
- Click the "Create form" button
- Add a name for the form and indicate your randomization settings, then click Save. A new form will be added, but you still have to place a test part and a section on it.
- Click the "Unplaced test parts" link on the right hand side to expand it.
- Click the edit (pencil) icon and click "Place"
- Click the "Unplaced sections" link on the right hand side to expand it.
- Click the edit (pencil) icon and click "Place"
- Add items to your section using the import option, random selection option, or by searching within the Linked Item Banks. Remember - items must be published before they can be added to an exam form.
*Note: You can switch between forms by selecting the appropriate form from the dropdown at the top of the Design tab. The first form created will display by default until it's removed.
